Neoadjuvant taxane plus trastuzumab and pertuzumab with or without carboplatin in HER2+ breast cancer

New results from the neoCARHP phase 3 trial, recently published on the Journal of Clinical Oncology show the non-inferiority of a neoadjuvant therapy with a selected taxane plus trastuzumab without carboplatin versus the same treatment with carboplatin in patients with stage II and stage III HER2+ breast cancer.
The neoCARHP trial was a multicenter, randomized, phase III, non-inferiority study that enrolled 774 patients with previously untreated, stage II and III, HER2-positive invasive breast cancer. Participants were randomized to receive a neoadjuvant therapy with investigator-selected taxane (docetaxel, paclitaxel, or nab-paclitaxel) plus trastuzumab and pertuzumab, with or without carboplatin. A pathologic complete response was achieved in 64.1% of patients not receiving carboplatin and in 65.9% of the group receiving carboplatin; the forst group had fewer grade 3 and 4 adverse events (20.7% versus 34.6% in patients treated with carboplatin). As authors point out, «The neoCARHP study demonstrated that the regimen without carboplatin achieved non-inferior pathologic complete response rates and improved tolerability. These findings suggest that omitting carboplatin may serve as an alternative neoadjuvant strategy in combination with dual HER2 blockade for patients with early-stage HER2-positive breast cancer». Thus, this trial broadens the therapeutic options for women with HER2-positive breast cancer.
